Provision of CMT Services to Individuals with Challenges
A Certified Medication Technician (CMT) is a member of the healthcare profession who is essential in the administration of medication to patients under the direction of a registered nurse or other qualified healthcare provider. CMT’s ensure the safe and accurate delivery of medications, thanks to their specialized training and knowledge in medication administration, which benefits individuals’ overall health and recovery in a variety of healthcare settings.
Responsibilities of a Certified Medication Technician

1. Medication Administration
CMT’s are in charge of giving patients their medications in accordance with doctor’s orders and recommended schedules. They meticulously adhere to established protocols to guarantee precise dosages, suitable administration routes and the right timing.

2. Medication Documentation
CMT’s keep precise and current medication records that include information on dosage, administration, and any side effects or reactions that may have been seen.  They make sure all pertinent information, including orders for medication changes and refills, is properly documented.

3. Patient monitoring
After administering medication, CMT’s keep an eye on their patients to look for any negative reactions, side effects, or changes in their condition. They alert the supervising nurse or healthcare provider right away if they have any worries or notice anything out of the ordinary.

4. Medication Safety
CMT’s follow stringent safety procedures to avoid medication errors. These procedures include double-checking medication labels, confirming patients’ identities, and using good hand hygiene.  They ensure that the area where medications are administered is secure and well-organized, and that medicines are handled, stored, and disposed of properly.

5. Collaboration and Communication
To ensure efficient communication and collaboration in medication administration and patient care, CMTs work closely with the healthcare team, which includes registered nurses, physicians, and pharmacists.

They let the supervising healthcare professionals know if there are modifications to medication orders, patient reactions, or worries.  CMT’s further assist with the planning and implementation of individuals’ daily social activities.  For example,  community rides, community development services (CDS), groceries, movie, games, visit to parks etc.
